Most people think of ringworm as a minor nuisance — an itchy, circular rash that clears up with a cream from the drugstore. But a new strain spreading rapidly across the world is turning that assumption on its head. The fungal pathogen Trichophyton indotineae is known for causing more serious cases of ringworm that are often resistant to common medications. And according to scientists, its global footprint is growing fast.

Data from the Mass Spectrometry Identification global network revealed a 3-fold increase in T. indotineae identification during 2022–2025, demonstrating that this network is an effective tool for monitoring global spread of this pathogen. That surge has alarmed researchers who say the fungus is no longer confined to the regions where it first appeared.

Origins and the Problem of Drug Resistance

Although the first cases of infection by T. indotineae were retrospectively identified as early as 2008, beginning in 2016 dermatologists in India began to report a sharp increase in cases of extensive and difficult-to-treat dermatophytosis. The situation in India escalated quickly. In a recent survey, the vast majority — 78% — of dermatophyte infections in that country were found to be caused by T. indotineae.

The core danger is resistance to the drugs doctors rely on most. Terbinafine, the first-line treatment for dermatophytosis, has limited alternatives, making resistance particularly concerning. Research has confirmed just how widespread that resistance is: susceptibility testing indicated that 63 — or 70% — of 90 T. indotineae isolates were resistant to terbinafine. Worse still, decreased susceptibility to azoles was also observed for some isolates, raising concern over the potential evolution of multi-drug resistance.

Its emergence may have been driven by the improper and excessive use of topical antifungals and corticosteroids, while other potential contributing factors include a lack of hygiene, overcrowding, working in hot and humid environments, and wearing tight-fitting synthetic clothing.

A Truly Global Spread

What began as a regional crisis has become a worldwide concern. Trichophyton indotineae emerged in South Asia and has spread rapidly over the past decade to the United States, South America, Australia, China, Canada, Iran, Japan, Kuwait, Turkey, Vietnam, and Europe. Cases have even been acquired locally within Europe, far from the original endemic zones.

Transmission is proving harder to contain than expected. Its anthropophilic pattern promotes inter-human transmission, and familial cases were detected in about half of patients, with the sharing of everyday objects particularly implicated. The fungus has also been linked to sexual transmission. Researchers reported four cases of sexually transmitted dermatophytosis caused by T. indotineae in Europe, highlighting the risk of the pathogen spreading through sexual contact outside its initial endemic areas.

What Comes Next

Treatment, when standard drugs fail, becomes a grueling ordeal. One case in Singapore showed limited response despite prolonged azole therapy, only resolving after combination therapy with anidulafungin and itraconazole. That kind of difficult, drawn-out treatment is what health experts fear could become increasingly common.

While not fatal, these infections are highly prevalent, affecting 20–25% of the global population and causing severe discomfort due to inflammation, itchy rashes, and scaling. Scientists are calling for urgent international cooperation. Researchers emphasize the importance of international collaboration to increase clinical recognition of the disease, access to modern molecular testing methods, appropriate treatment, and judicious use of antifungals. As drug-resistant fungi continue to evolve and travel, the story of T. indotineae serves as a stark reminder that even the most ordinary infections can become serious public health threats when the tools we use to fight them stop working.
