Against that sobering backdrop, a new study out of Emory University is offering a rare reason for cautious optimism — and the potential intervention costs just a few cents a day.
Published in the journal Sleep Medicine, the study found that daily intake of a vitamin D supplement was linked with a significant boost in cognitive test results for people with two hallmarks of early dementia: mild cognitive impairment (MCI) and sleep disturbances. This preliminary study is the first to examine how vitamin D supplement intake relates to cognitive function specifically in a high-risk group experiencing both MCI and sleep disturbances.
The study found that participants who reported taking 5,000 IU or more of vitamin D daily scored more than 13% higher on the Montreal Cognitive Assessment (MoCA) than those who didn't, even after accounting for other factors. The MoCA is a commonly used screening tool that assesses memory, attention, language, and other thinking abilities. That's a meaningful gap on a test specifically designed to flag dementia risk.
Lower daily doses of vitamin D were not associated with higher cognitive scores. That detail matters. It suggests the effect isn't simply about taking any vitamin D — the dose appears to be the critical variable. The study also found no significant difference between vitamin D2 and vitamin D3, suggesting that dose may be more important than the type of supplement consumed.
Sleep problems are also common among people with MCI, affecting nearly half of patients and potentially increasing the risk of further cognitive decline. Vitamin D deficiency has previously been associated with sleep problems including insomnia and more frequent nighttime awakenings. That overlap may help explain why this particular group of patients showed such a notable association.
MCI is an intermediate stage between normal cognitive aging and dementia. Researchers stress that catching patients at this stage — before a full dementia diagnosis — could be the key to any intervention's effectiveness. As study senior author Victoria Pak put it, "this may represent a critical window for intervention, when cognitive changes are emerging, but opportunities to support brain health may remain."
Pak, an associate professor at Emory University's Nell Hodgson Woodruff School of Nursing, added that "identifying accessible and modifiable factors, such as vitamin D supplement intake, during the earlier stages of cognitive decline may become increasingly important, particularly as rates of Alzheimer's disease continue to rise." The word "accessible" is doing a lot of work there — vitamin D supplements are widely available without a prescription and are among the most affordable supplements on the market.
The study included just 54 adults with sleep disturbances and mild cognitive impairments. That's a small sample, and researchers are careful to frame this as a preliminary finding. The findings do not prove that vitamin D improves memory or prevents dementia. It is possible that people with better cognitive function are also more likely to engage in health-conscious behaviors, including taking supplements.
Researchers emphasize that larger follow-up studies are necessary to confirm the results, including assessments of vitamin D toxicity, as current upper intake level recommendations are below 4,000 IU per day. That's a crucial point — 5,000 IU exceeds standard guidelines, and anyone considering high-dose supplementation should consult a physician first. Still, the preliminary findings suggest vitamin D could be worth studying as a potentially modifiable factor during an early window of cognitive decline.
