Finn's Take· TL;DRMost people associate Alzheimer's disease with old age — something to worry about later, if at all. But a compelling new study suggests the story may begin much earlier, quietly unfolding in your 30s and 40s, and that something as accessible as vitamin D could play a meaningful role in how it ends.
People with higher levels of vitamin D in midlife may have lower levels of tau protein in the brain years later, according to a study published April 1, 2026, in *Neurology Open Access*, an official journal of the American Academy of Neurology. Buildup of tau can lead to the formation of spaghetti-like tangles that choke off healthy brain cells, speeding the onset of dementia.
The authors conducted a prospective cohort study drawing on 793 dementia-free participants from the Framingham Heart Study Generation 3 cohort, with participants having their vitamin D levels measured at a mean age of 39 years. The research drew on the Framingham Heart Study, the longest running longitudinal cohort study in the world, which began in 1948 with 5,200 adults from Framingham, Massachusetts.
About 16 years later, around 430 of the same group participated in brain positron emission tomography (PET) scans, assessing them for signs of tau or amyloid beta, protein clumps long implicated in Alzheimer's disease. Researchers found that individuals with higher circulating levels of vitamin D had lower accumulations of tau when scanned roughly 16 years later — and the association held even after adjusting for a wide range of factors, including age, sex, cardiovascular health, smoking, depression, and body mass index.
While the study found no relationship between vitamin D and amyloid-beta, a link between vitamin D and tau became apparent — and this applied to both the brain overall, as well as some of the regions known to be affected by Alzheimer's in its earliest stages. Notably, 34% of participants had vitamin D levels below the study-defined threshold of 30 ng/mL, while only 5% were on vitamin D supplementation.
"Vitamin D in mid-life could be an important factor to protect future brain health," said neurologist and senior author Emer McGrath from the University of Galway in Ireland, noting that "while previous research has linked low vitamin D in adults over 70 with an increased risk of dementia, this study is among the first to look at younger adults at mid-life, around the average age of 39."
Co-author Martin David Mulligan noted that "mid-life is a time where risk factor modification can have a greater impact," adding that higher vitamin D levels in midlife may offer protection against developing tau deposits and that low vitamin D could potentially be a modifiable risk factor to reduce the risk of dementia.
The researchers note that the findings show a relationship, not proof that vitamin D directly reduces tau levels or lowers the risk of dementia. Outside experts echoed that caution: "The strongest evidence still supports a multifactorial prevention approach, not a single nutrient or intervention," said one physician, adding that "vitamin D may play a role in overall health, but it should be viewed as one small piece of a much larger prevention strategy."
The researchers themselves wrote that "supplementation with higher doses of vitamin D, and/or over longer periods of time in younger, cognitively healthy individuals may be beneficial, as the window of opportunity for disease modification is greater" — but stressed that "this will require formal testing in clinical trials." Still, in a field where so few modifiable risk factors have shown promise this early in life, the findings add real momentum to the case for keeping your vitamin D levels in check — not just for your bones, but potentially for your brain, decades down the road.
