A Gut-Level Breakthrough for Allergy Sufferers

Some 33 million Americans live with severe food allergies — and for many of them, a single accidental bite of peanut can be life-threatening. Now, a groundbreaking study out of Boston Children's Hospital is pointing toward a potential cure that, while unconventional, could change everything: capsules filled with the feces of healthy, non-allergic donors.

In a small clinical trial, Dr. Rima Rachid, director of the Food Allergy Program and the Allergen Immunotherapy Program, and Dr. Talal Chatila, director of translational immunology, and their colleagues found that some participants with peanut allergies experienced a higher tolerance to peanuts months after receiving a fecal transplant from a healthy donor without food allergies. The landmark study is the first to demonstrate that a microbiome-based therapy may improve food allergies in people while also revealing how gut bacteria, their metabolites, and the immune system work together to influence treatment response.

What the Trial Actually Involved

Researchers recruited 10 young adults with severe peanut allergies who reacted to mere traces of peanuts and had them swallow frozen "poop pills" containing feces — including strains of helpful bacteria — from donors with healthy gut microbiomes. Participants swallowed 36 capsules containing FMT prepared from stool donated by healthy, nonatopic donors through the nonprofit stool bank OpenBiome.

Three months later, three participants could tolerate eating multiple peanuts. In the next round of the study, another five volunteers took antibiotics before the fecal transplant to see if the donor bacteria transferred more successfully when they didn't have to compete with the participants' own resident gut microbes — and three of those five also improved in peanut tolerance, ingesting more than four peanuts before reacting. That's a meaningful signal for a Phase 1 trial, which is designed primarily to test safety, not effectiveness.

The Bile Acid Connection

Perhaps the most significant finding wasn't just that some patients improved — it was *why* they improved. Researchers found evidence that bile acid metabolism may connect the altered gut microbiome to improved immune tolerance. Human participants who responded to FMT had increased levels of bile salts in their blood compared with non-responders, supporting a link between the donor bacteria's ability to process bile acids and the development of immune cells that protect against allergic reactions.

To test that mechanism more directly, researchers engineered a strain of *Bacteroides ovatus* without a gene needed to produce bile salt hydrolase, an enzyme involved in bile acid processing. In allergic mice, the modified bacteria could no longer promote protective immune cells or prevent anaphylaxis — providing strong evidence that bacterial bile acid metabolism was necessary for the protective effect. In plain terms: the right gut bacteria appear to retrain the immune system to stop overreacting to peanuts.

What Comes Next

While the study's findings show promise, FMT cannot yet be recommended as a treatment for allergies. But the research team is already pushing forward. Dr. Rachid is now leading a new study in which teens will receive microbiota transplantation therapy — a purified, concentrated form of microbes rather than poop pills — that can be stored in a home refrigerator. Patients will take the capsules at home under medical supervision and then undergo food challenges to evaluate changes in their tolerance to peanuts.

These data show how reshaping the gut microbiome can blunt food allergy, and may even point to a potential cure, according to Dr. Rachid. "This is an important and exciting study because it provides proof of concept that modulating the microbiome can protect against food allergy in patients with this disease," said Dr. Cathryn Nagler of the University of Chicago, who was not involved in the study. For the millions of families navigating the daily anxiety of life-threatening nut allergies, that proof of concept — however early — represents a genuine reason for hope.
