Most people in their 20s aren't thinking about heart disease. They're young, they feel fine, and cardiovascular trouble seems like a problem for later in life. But a striking new study published on August 20, 2026, in Circulation: Population Health and Outcomes is upending that assumption — and the numbers are hard to ignore.

A new scientific study found that nearly 8 out of 10 young adult participants, with an average age of just 23 years, had cardiovascular-kidney-metabolic (CKM) syndrome, according to research published in a peer-reviewed journal of the American Heart Association. Participants with more advanced CKM syndrome had early signs of arterial injury, the researchers report, highlighting the urgent need for cardiovascular disease prevention efforts to begin earlier in life.

When researchers imaged and measured the carotid arteries of more than a thousand young participants, they noticed signs of subclinical injury — with some markers of arterial thickness consistent with what would be expected in a 33-year-old, despite the average participant age being just 23. That's a decade of invisible aging, already baked into the arteries of people who likely consider themselves perfectly healthy.

What Is CKM Syndrome — and Why Does It Matter?

Scientists analyzed health data from 1,283 participants in the Future of Families–Cardiovascular Health Among Young Adults (FF-CHAYA) study, a diverse cohort with an average age of 23 years, and found that nearly 80% met criteria for CKM syndrome stage 1, 2, or 3 — meaning they already had excess body fat, metabolic risk factors, and/or evidence of subclinical cardiovascular disease.

Stage 1 was identified in 40.2% of participants, reflecting excess body fat, while 36.2% were at Stage 2 with metabolic risk factors. Stage 3, indicating subclinical cardiovascular disease or advanced kidney disease, was present in 2.8% of participants. Only about 21% had no identifiable CKM risk factors at all. In other words, the overwhelming majority of young adults in the study were already on a troubling trajectory.

More advanced CKM syndrome stages were associated with early arterial injury, including thicker carotid artery walls — an early marker of atherosclerosis — even among people in their early 20s. Cardiovascular health was measured using Life's Essential 8, covering diet, physical activity, sleep, nicotine exposure, BMI, blood glucose, blood lipids, and blood pressure. Life's Essential 8 scores declined sharply across CKM stages, from 77.6 in stage 0 to just 64.4 in stage 3.

The Bigger Picture: Prevention Must Start Sooner

Traditional cardiovascular risk factors — elevated cholesterol, blood pressure, diabetes, and smoking — have long been linked to arterial damage in older adults, and even childhood levels of these risk factors, within the normal range, predict later arterial injury. This study reinforces that the clock starts ticking far earlier than medicine has historically acknowledged.

The study shows that CKM syndrome staging and Life's Essential 8 are "complementary for identifying adults younger than age 30 who may be on the fast track for arterial injury, plaque build-up and, ultimately, at higher risk for future" cardiovascular events. Researchers say this points to a clear need to rethink when and how doctors screen for heart risk — not in middle age, but in young adulthood.

The study does have limitations, including its cross-sectional design, which shows association but cannot establish cause and effect, and because the study population intentionally included many individuals from disadvantaged backgrounds, the findings may not be generalizable to all young adults. Researchers say additional long-term studies are needed to determine how CKM syndrome stage in young adulthood relates to future cardiovascular disease risk. Still, the signal is clear: waiting until middle age to address heart health may already be too late. The arteries don't wait — and neither should we.
