An Alarming Finding Above the Clouds

Of more than 500 occupations in the United States, flight attendants and pilots have the highest and second-highest proportions of radiation-related cancer deaths, a new study has found. The research, published on August 17, 2026, in JAMA Internal Medicine by a team from Harvard Medical School, delivers a striking verdict: the people keeping commercial aviation running face a cancer burden unlike any other workforce in the country — including those who work directly with nuclear materials.

The study, led by Vishal Patel, HMS clinical fellow in surgery at Brigham and Women's Hospital, used the National Vital Statistics System, looking at all U.S. death certificates from 2020 to 2024. These records have only recently been linked to the occupation of the decedent, yielding data from nearly 13 million decedents across 503 occupations, including 14,000 pilots and 7,000 flight attendants.

Outpacing Even Nuclear Workers

About 6.9 percent of deaths among flight attendants and 6.7 percent of deaths among pilots were from radiation-related cancers, according to the analysis. That may sound like a small number, but context makes it alarming. Flight attendants were 50 percent more likely to experience radiation-related cancer than other occupation groups in the analysis, and pilots were 36 percent more likely. Most strikingly, airline pilots and attendants were significantly more likely to die from cancers related to radiation exposure than other professions, even compared to jobs where people have to intentionally spend time near radiation, like nuclear technologists.

Research shows that aircrew members receive the largest annual effective dose of ionizing radiation of any workforce in the United States, largely because of exposure to cosmic radiation at high altitudes. Commercial planes cruise above 30,000 feet, where a thinner atmosphere offers far less shielding from high-energy particles streaming in from the sun and deep space. That radiation can damage DNA and elevate the risk of cancers including leukemia, breast cancer, lymphoma, thyroid cancer, and multiple myeloma. For cancers unrelated to radiation, aircrew sat near the middle of the pack — and ground-based aviation workers did not show elevated risk, exactly as the radiation hypothesis predicted.

Frequent Flyers: Don't Panic

For the millions of people who board planes regularly, the lead researcher offered reassurance. An air traveler who takes 10 cross-country roundtrips annually would be exposed to 0.4 millisieverts of cosmic radiation — far below the estimated 3 to 6 millisieverts of cosmic radiation to which pilots and flight attendants are exposed each year, depending on the flight paths they follow. "This really is a story about cumulative occupational exposure, not just routine civilian travel," Patel said.

Patel does note that their work can't completely rule out other factors common to aircrew employees that might be raising their cancer risk, particularly the frequent disruption of their circadian rhythm from flying long-haul and overnight flights. Still, the internal logic of the data is hard to dismiss. The cancers showing up are precisely the ones radiation is known to cause, and they're concentrated in exactly the workers most exposed to it.

A Regulatory Gap That Needs Closing

The Federal Aviation Administration formally recognizes pilots and flight attendants as occupationally exposed to ionizing radiation, but unlike other radiation-exposed workers or aircrew in other countries, U.S. aircrew are not subject to federal dose limits or monitoring requirements. That gap stands in stark contrast to how the U.S. treats nuclear industry workers, whose radiation exposure is carefully tracked and capped.

The results support considering occupational radiation protections for U.S. aircrew members commensurate with their level of exposure, said senior author Anupam Jena, the Joseph P. Newhouse Professor of Health Care Policy in the Blavatnik Institute at Harvard Medical School. This research clearly indicates that pilots and flight attendants need better protection from ionizing radiation while on the job, though trying to build inherently more protective airplanes won't work, Patel says. Whether that means mandatory monitoring, adjusted flight schedules, or new federal dose limits, the study makes one thing clear: the people who get us where we're going deserve the same protections we already extend to workers on the ground.
