Finn's Take· TL;DRA sweeping new study has added powerful evidence to one of the most pressing questions in women's health: can hormone replacement therapy protect the aging brain? The answer, researchers now say, may hinge almost entirely on timing. Hormone replacement therapy reduces women's likelihood of developing Alzheimer's disease — in some cases dramatically — but only when started before the age of 56, according to a study that included more than 180,000 UK Biobank participants.
The study, published in the journal Alzheimer's & Dementia, analyzed UK Biobank data from 183,450 postmenopausal women with an average age of 60. Researchers tracked the participants for an average of more than 13 years. They determined that HRT use was associated with about a 10% lower risk of all-cause dementia and a 16% lower risk of Alzheimer's disease over that follow-up period. The 23% figure making headlines applies specifically to women who began therapy during the most beneficial age window — a detail that makes the finding even more actionable.
The protective association was strongest among women who started therapy between the ages of 46 and 56, and also high among those who had undergone surgical menopause. Starting HRT after age 56 was not associated with any reduced dementia risk. That cutoff is striking. It suggests the brain may be most receptive to estrogen's protective effects during and shortly after the menopausal transition — not years later.
Two groups of women stood out in particular: those who went through surgical menopause — meaning a hysterectomy and/or removal of both ovaries — and those who carry a gene variant that raises lifetime risk of Alzheimer's. For both groups, HRT use came with a notably stronger association with lower dementia risk. Women who had undergone surgical menopause and used HRT had a 32% lower Alzheimer's risk. Surgical menopause causes a more abrupt drop in estrogen , which may explain why replacing that hormone earlier and more aggressively appears to matter so much for this group.
Women are around twice as likely to get Alzheimer's disease — the most common type of dementia — in old age as men are. The reasons for this are unclear, but there are hints that menopause has something to do with it. Estrogen supports brain function, including communication between brain cells and blood flow. The timing findings fit the "window of opportunity" idea, which suggests HRT may have more potential to support brain health when started during perimenopause or the early postmenopausal years rather than years later.
Not all forms of HRT carry equal risk profiles, either. For oral versions of estrogen, there may be an increased risk of blood clots — a risk that does not appear to exist for transdermal options, such as patches, gels, and creams. Researchers were also careful to flag the study's limits: women who use HRT may differ from non-users in other ways that affect dementia risk, such as their overall health, lifestyle, or access to healthcare.
As this was an observational study, it could not establish that HRT prevents dementia — only that an association exists. The UK researchers plan to conduct randomized controlled trials to confirm that HRT reduces dementia risk, with future studies examining factors such as estrogen-only versus combined therapy, dosage amounts, administration route, treatment duration, age at the start of therapy, menopause type, and potentially APOE genotype.
The researchers suggest that different subgroups of women may respond differently to HRT, justifying future randomized controlled trials to provide evidence that could help frame more tailored HRT prescribing guidelines in order to reduce dementia risk. For millions of women navigating menopause decisions right now, the message from this research is clear: the conversation about hormone therapy may be far more consequential — and far more time-sensitive — than previously understood.
